Tailbone (sacrum and coccyx) X-ray
Quick answer: A sacrococcygeal X-ray shows the sacrum and coccyx, the bones at the very bottom of the spine. Doctors usually order it for tailbone pain, especially after a fall onto the bottom.
- SampleImaging (no sample)
- TakesAbout 10 minutes
- Before the testNo special preparation is usually needed, but remove metal items from the area and tell staff if you might be pregnant.
How the test is done
- 1You change into a gown and remove anything metal from around your hips

- 3For the side view you lie on your side with knees slightly bent
- 4You may be asked to empty your bladder first so the bones show more clearly
What your results can mean
Reading your report
A radiologist reports on the images, and your doctor will explain what the findings mean for you.
If the result is outside the usual range
A tailbone X-ray can show a fracture or dislocation of the coccyx or sacrum, or a tailbone that sits at an unusual angle. Many people with tailbone pain have a normal X-ray.
Who usually has this test
People with pain at the base of the spine on sitting, especially after a fall onto the bottom or after childbirth.
Risks and aftercare
Lying on a sore tailbone can be uncomfortable for a few minutes; because the beam is close to the pelvis, always tell staff if you might be pregnant.

Questions about this test
Can tailbone pain happen after childbirth?
Yes, the tailbone can be bruised or strained during vaginal birth. The pain usually settles over weeks to months.
What if my tailbone X-ray is normal but it still hurts?
Bruising and strain of the area often do not show on X-ray. Cushions, avoiding long sitting and pain relief advised by your doctor often help it settle.
Why am I asked to empty my bladder before a tailbone X-ray?
A full bladder and bowel can hide the small tailbone bones. Emptying first gives a clearer picture.
