Transrectal ultrasound (TRUS)

How the test is done

  1. 1Follow any bowel preparation you are given, such as an enema
  2. 2You lie on your left side with knees drawn up towards your chest
  3. 3A lubricated, covered probe about the width of a finger is placed into the back passage
  4. 4The probe is moved gently to view the prostate and seminal vesicles

What your results can mean

Reading your report

A sonologist or radiologist reports on the scan, and your doctor will explain what the findings mean for you.

If the result is outside the usual range

A TRUS can show an enlarged prostate, a cyst or blockage of the ejaculatory ducts, swollen seminal vesicles, or areas that may need further tests. In men with very low semen volume or no sperm, it helps look for a blockage.

Who usually has this test

Men with prostate symptoms or a raised PSA, and men with a very low semen volume or no sperm on semen analysis.

Risks and aftercare

There is no radiation; you may feel pressure and an urge to open your bowels during the scan, and slight spotting from the back passage can happen.

Questions about this test

Why is a TRUS done for male infertility?

In some men with very low semen volume or no sperm, a blockage near the prostate stops sperm getting through. TRUS can show blocked ducts or cysts that may be treatable.

Does a TRUS hurt?

Most men feel pressure and discomfort rather than pain. Breathing slowly and staying relaxed helps, and you can ask the doctor to pause.

Is a TRUS the same as a prostate biopsy?

No, a TRUS alone only takes pictures. A biopsy takes tissue samples and is a separate procedure, though it may use the same probe to guide it.
