Estradiol (E2) blood test
Quick answer: An estradiol test measures E2, the main form of oestrogen made by the ovaries. Doctors order it to check ovarian function, irregular periods or menopause, and to monitor follicle growth during fertility treatment.
- SampleBlood
- TakesAbout 5 minutes
- Before the testYour doctor may ask for the sample on a particular cycle day, often day 2 or 3, or on monitoring days during treatment.
- In fertility careDuring IVF stimulation, estradiol is tracked with follicular scans to judge the response and time the trigger injection.
How the test is done
- 1You come on the cycle day or monitoring day your doctor gives
- 2The inside of your elbow is cleaned
- 3A needle draws blood into a tube
- 4During IVF, results are read with your follicle scan that day
What your results can mean
Reading your report
Estradiol changes greatly across the cycle and during treatment, so discuss your result with your doctor.
If the result is outside the usual range
A high level can be seen during IVF stimulation, with ovarian cysts or some ovary conditions. A low level can be seen with menopause, low ovarian reserve, very low body weight or pituitary problems.
Who usually has this test
Women with irregular or absent periods, suspected menopause, and women having fertility treatment such as IVF or ovulation induction.
Risks and aftercare
There is little to do after the test; during treatment, follow your clinic's dosing plan for that day.

Questions about this test
Why is estradiol checked during IVF?
Estradiol rises as follicles grow, so it helps your doctor judge how your ovaries are responding to the medicines. Together with scans it helps decide doses and the timing of the trigger injection.
Why is estradiol tested on day 2 or 3 of the period?
Early in the cycle, estradiol should be low. A high day-2 or day-3 level can hide a raised FSH and may suggest lower ovarian reserve or a cyst.
Can a very high estradiol in IVF be a concern?
A very high level can go with many follicles and a higher chance of ovarian hyperstimulation syndrome. Your doctor may adjust your plan to lower that risk.
