Pleural fluid analysis
Quick answer: Pleural fluid analysis tests fluid taken from the space around the lungs, looking at its cells and make-up to find the cause of the fluid. Doctors order it when a chest X-ray or scan shows fluid around the lungs, for example from infection, tuberculosis, heart failure or cancer.

- TakesAbout 15 to 30 minutes
- Before the testThe fluid is collected by a doctor with a needle, who will tell you how to prepare.
How the test is done
- 1You sit on the edge of a bed, leaning forward on a table or pillow
- 2The skin on your back is cleaned and numbed with an injection
- 3A needle or thin tube is passed between the ribs into the fluid, often with ultrasound
- 4Fluid is drawn into tubes or a bag, and you may feel pressure or an urge to cough
- 5A small dressing is put on, and a chest X-ray may be done afterwards
What your results can mean
Reading your report
The lab results are read together with your symptoms and scans; your doctor will explain what they mean.
If the result is outside the usual range
The tests can show whether the fluid is from leaking fluid, as in heart failure, or from a lung or lining problem such as infection, tuberculosis or cancer. Raised protein, white cells, a high ADA level or abnormal cells each point in different directions, and your doctor reads them together.
Who usually has this test
People with fluid around the lungs seen on an X-ray or scan, often with breathlessness, cough or fever.
Risks and aftercare
Soreness at the needle site and coughing are common; seek urgent help for sudden breathlessness, chest pain or coughing up blood.

Questions about this test
Can pleural fluid show tuberculosis?
Pleural fluid tests can give strong clues, such as a high ADA level and certain white cells. TB in the fluid is not always proven, and sometimes a biopsy of the lining is needed.
Is a pleural tap painful?
The numbing injection may sting, and you may feel pressure as the needle goes in. Most people find the procedure uncomfortable rather than very painful.
Why do I need a chest X-ray after the tap?
It checks that the lung has not been punctured and that fluid has been removed. Doctors may do it if you feel breathless afterwards.
