Body fluid cytology (ascitic fluid, CSF and other fluids)

Quick answer: Fluid cytology looks at cells in a body fluid under a microscope to check for infection, inflammation or abnormal (cancer) cells. Doctors order it when fluid builds up in the belly or elsewhere, or when spinal fluid is tested, to help find the cause.

How the test is done

  1. 1The doctor explains the procedure and the skin over the fluid is cleaned
  2. 2Numbing medicine may be injected into the skin first
  3. 3A needle is passed into the fluid, sometimes guided by ultrasound, and fluid is drawn off
  4. 4For spinal fluid you lie curled on your side while a needle goes into the lower back
  5. 5The fluid is sent to the lab, where cells are spread on slides and examined

What your results can mean

Reading your report

A pathologist examines the cells under a microscope and writes a report; discuss what it means for you with your doctor.

If the result is outside the usual range

The cells can show signs of infection, such as many white cells, inflammation, or abnormal cells that can point to cancer. A result with no abnormal cells does not fully rule out cancer, and other fluid tests are usually done as well.

Who usually has this test

People with fluid building up in the belly, around the lungs or heart, or people having spinal fluid tested for infection or other causes.

Risks and aftercare

You may feel soreness at the needle site; after a spinal tap, lying flat and drinking fluids may help a headache, and seek help for fever, increasing pain or leaking.

Questions about this test

What is ascitic fluid?

Ascitic fluid is fluid that collects in the belly, often from liver disease, infection, heart failure or cancer. Testing it helps find the cause.

Will removing fluid make me feel better?

When a lot of fluid is drawn off, breathing or belly discomfort can ease. Fluid may build up again until the cause is treated.

What does malignant cells seen mean?

It means cells in the fluid look like cancer cells. Your doctor will arrange further tests to find where they came from and plan treatment.
