Platelet indices (MPV, PDW, P-LCR)

Quick answer: Platelet indices describe the size of your platelets and how much their size varies. They help doctors understand the cause of a low or high platelet count, alongside the platelet count itself.

How the test is done

  1. 1Blood is drawn into an EDTA tube
  2. 2The analyser measures platelet sizes as it counts them
  3. 3MPV, PDW and P-LCR are worked out
  4. 4They are reported alongside the platelet count

What your results can mean

Reading your report

These values are rarely meaningful on their own; your doctor will read them with your platelet count.

If the result is outside the usual range

Larger platelets (high MPV) can be seen when the marrow is making new platelets quickly, as in immune thrombocytopenia. Small platelets with a low count can point to reduced production in the marrow.

Who usually has this test

People with a high or low platelet count, to help find the cause.

Risks and aftercare

Bruising is possible, and more likely if your platelet count is low.

Names and options

Also called: MPV, PDW, PLCR, mean platelet volume

Options

  • MPV (mean platelet volume)
  • PDW (platelet distribution width)
  • P-LCR (platelet large cell ratio)

Listed by the hospital as

  • MPV (MEAN PLATELET VOLUME)
  • PDW (PLATELET DISTRIBUTION WIDTH)
  • PLCR (PLATELET LARGE CELL RATIO)

Questions about this test

Is a high MPV a disease?

No, it is a clue rather than a diagnosis. Doctors read it with the platelet count and other tests.

Can a delay in testing affect platelet indices?

Yes, platelets swell in the tube over time, which raises the MPV. Labs try to test samples promptly.

Why are platelet indices sometimes not reported?

When the platelet count is very low or the platelets have clumped, the analyser may not give reliable indices. A smear may be checked instead.
