Iron binding tests (TIBC, UIBC, transferrin saturation)

Quick answer: These blood tests show how much iron your blood can carry and how much of that capacity is being used. They are ordered with serum iron and ferritin to look into anaemia, low iron or iron overload.

  • Before the testA morning sample is often preferred; follow the lab's instructions.

How the test is done

  1. 1A morning sample is often preferred, as iron levels vary through the day
  2. 2The skin over a vein is cleaned
  3. 3Blood is drawn into a tube, usually with serum iron and ferritin
  4. 4The lab works out TIBC, UIBC and transferrin saturation

What your results can mean

Reading your report

Normal ranges differ between labs, so compare your result with the range printed on your report and discuss it with your doctor.

If the result is outside the usual range

A high TIBC with low saturation can point to iron deficiency, and is also seen in pregnancy or with hormone pills. A low TIBC with high saturation can point to iron overload, while a low TIBC can also be seen with long-term illness or poor nutrition.

Who usually has this test

People with anaemia, tiredness, heavy periods, or suspected iron overload, and women before or during pregnancy.

Risks and aftercare

You can take your iron tablet after the sample if your doctor has told you to do so.

Questions about this test

What is transferrin saturation?

Transferrin saturation shows what percentage of the iron-carrying protein is loaded with iron. A low percentage suggests iron deficiency, while a high percentage can suggest iron overload.

Should I stop iron tablets before this test?

Iron tablets can affect the result. Ask your doctor whether to skip your dose on the morning of the test, but do not stop treatment on your own.

Why is ferritin tested with TIBC?

Ferritin shows how much iron is stored, while TIBC and saturation show how much is being carried in the blood. Together they give a clearer picture of your iron status.
