Fluid microscopy for crystals
Quick answer: This test looks under a microscope for crystals in body fluid, usually fluid taken from a swollen joint. It helps doctors tell gout (uric acid crystals) apart from pseudogout and other causes of joint swelling.
- SampleFluid
- TakesAbout 10 to 20 minutes for the fluid to be drawn
- Before the testThe fluid is collected by a doctor, who will explain any preparation.
How the test is done
- 1A doctor cleans the skin over the swollen joint
- 2A numbing injection may be given first
- 3A needle draws a small amount of fluid into a syringe
- 4The lab looks at the fluid under a special polarised light microscope
What your results can mean
Reading your report
Results name the type of crystals seen, if any; your doctor will explain what they mean for treatment.
If the result is outside the usual range
Needle-shaped uric acid crystals can point to gout, and other crystals can point to pseudogout (calcium pyrophosphate disease). Finding no crystals does not fully rule out these conditions, and infection is checked separately.
Who usually has this test
People with a sudden painful, hot, swollen joint, where gout, pseudogout or infection is suspected.
Risks and aftercare
The joint may feel sore for a day or two; report fever, redness or rising pain, as there is a small risk of infection.
Names and options
Also called: joint fluid crystals, synovial fluid crystal test

Questions about this test
Can this test confirm gout?
Finding uric acid crystals in joint fluid is the clearest way to confirm gout. A blood uric acid level alone cannot confirm or rule it out.
Does drawing fluid from a joint hurt?
You may feel pressure and some discomfort, but a numbing injection is often used. Removing fluid can also ease the pain of a tight, swollen joint.
Why is the fluid also checked for infection?
An infected joint can look like gout and needs urgent treatment. Labs often do a cell count and culture on the same fluid.
