  2. 2A vein inside your elbow is cleaned
  3. 3A needle draws a tube of blood
  4. 4The lab runs an automated test for total treponemal antibodies

What your results can mean

Reading your report

This antibody often stays positive after treatment, so a positive result is read with a VDRL or RPR test by your doctor.

If the result is outside the usual range

A positive result shows infection with syphilis at some time, current or past, and is usually followed by a VDRL or RPR to check activity. A negative result usually rules out syphilis unless exposure was very recent.

Who usually has this test

People being screened for syphilis, including in pregnancy and before IVF, and people confirming a reactive VDRL.

Risks and aftercare

A small bruise can appear; if the result is positive, your doctor will also talk about partner testing.

Questions about this test

How is this test different from VDRL?

This test looks for antibodies to the syphilis bacterium itself, so it is more specific. VDRL measures a less specific antibody but is useful for following treatment.

Why do some labs screen with a treponemal test first?

Automated treponemal tests are sensitive and suit large numbers of samples. A positive result is then followed by a VDRL or RPR.

Does a positive result mean I need treatment again?

Not always. If you were treated before, the antibody often stays positive. Your doctor looks at the VDRL titre and history to decide.
