Islet cell antibody (IA-2 / ICA-512) test

Quick answer: This test looks for antibodies against IA-2 (also called ICA-512), a protein in the insulin-making cells of the pancreas. It helps doctors confirm type 1 or autoimmune diabetes, often together with a GAD65 antibody test.

How the test is done

  1. 1A small blood sample is taken from a vein
  2. 2The sample goes to a lab that runs islet antibody tests
  3. 3Antibodies against IA-2 are measured
  4. 4Your doctor compares the result with GAD65 and other results

What your results can mean

Reading your report

A positive result suggests autoimmune diabetes; discuss what it means with your doctor.

If the result is outside the usual range

A positive result can point to type 1 diabetes and is more common in children and young adults. Having several islet antibodies together suggests a higher chance of type 1 diabetes.

Who usually has this test

Children and adults with newly found diabetes where type 1 is suspected.

Risks and aftercare

Bruising is possible; keep checking your sugar as advised by your doctor.

Questions about this test

Why test more than one islet antibody?

Each antibody is found in only some people with type 1 diabetes. Testing several together increases the chance of finding one.

How does IA-2 differ from GAD65?

IA-2 antibodies are more common in children and tend to appear closer to the start of diabetes. GAD65 antibodies are more common in adults with slow-onset forms.

Can a positive result change treatment?

It can help doctors judge whether insulin is likely to be needed early. Treatment is still based on your blood sugar and overall picture.
