Mid and lower back (thoracolumbar spine) X-ray
Quick answer: A thoracolumbar spine X-ray shows the bones where the middle back meets the lower back, and their alignment. Doctors order it for back pain, injury, or to check spine curves such as scoliosis. A standing view shows the spine under body weight.
- SampleImaging (no sample)
- TakesAbout 10 to 15 minutes
- Before the testNo special preparation is usually needed, but remove metal items from the area and tell staff if you might be pregnant.
How the test is done
- 1You change into a gown and remove anything metal from your back and waist
- 2You stand or lie with your back against the plate for the front view
- 3For the side view you turn side-on, with arms raised or folded
- 4For scoliosis checks a standing picture of the whole spine may be taken
What your results can mean
Reading your report
A radiologist reports on the images, and your doctor will explain what the findings mean for you.
If the result is outside the usual range
The X-ray can show fractures, including compression fractures from thin bones, sideways curves (scoliosis), a forward curve (kyphosis), or wear changes. Discs and nerves do not show on X-ray.
Who usually has this test
People with mid or lower back pain after injury, older people with sudden back pain, and children or adults being checked for scoliosis.
Risks and aftercare
Standing still with arms raised can be tiring if your back is sore; this area gets more radiation than a limb X-ray, so tell staff if you might be pregnant.

Questions about this test
Can a back X-ray show osteoporosis?
An X-ray can show a fracture caused by thin bones, but it is not a good test for measuring bone strength. A bone density scan (DEXA) is used for that.
How is scoliosis measured on X-ray?
The doctor measures the angle of the curve on a standing X-ray of the spine, called the Cobb angle. Repeat X-rays show whether the curve is changing.
What is a compression fracture?
It is when a spine bone collapses and becomes wedge-shaped, often from weak bones in older people. It can cause sudden back pain and loss of height.
