Paracetamol (acetaminophen) blood level
Quick answer: This test measures the amount of paracetamol (acetaminophen) in your blood. It is mainly used when too much paracetamol may have been taken, to judge the risk of liver damage and guide treatment.
- SampleBlood
- TakesAbout 5 minutes
- Before the testNo special preparation is needed; tell staff the time and amount of the last dose if known.
How the test is done
- 1A nurse draws blood, ideally at least four hours after the dose was taken
- 2The time of the dose is noted on the form
- 3The lab measures paracetamol in the blood
- 4Doctors compare the level with the time since the dose
What your results can mean
Reading your report
The result is read against the time since the dose was taken; your doctor will explain what it means.
If the result is outside the usual range
A high level after an overdose can point to a risk of liver damage and helps doctors decide on treatment. A low or undetectable level means little paracetamol is in the blood at that time.
Who usually has this test
People who may have taken too much paracetamol, on purpose or by accident, and some people with liver problems.
Risks and aftercare
Bruising is possible; if the test is for an overdose, follow the emergency team's advice closely.

Questions about this test
Why wait four hours after an overdose to test?
Paracetamol takes time to be absorbed, so levels taken earlier can be misleading. Treatment is not delayed if the timing is unclear or the dose is large.
What should I do if someone has taken too much paracetamol?
Go to the emergency department straight away, even if they feel well. Early treatment works well, and symptoms often appear only later.
Does a low level mean no harm was done?
A level below the treatment line at the right time usually means a low risk of liver damage. Doctors may still check liver tests if the timing or amount is uncertain.
